BASKETBALL REGISTRATION FEES, DATES & TIMES Participation Fee $75.00 Facility Rental $25.00 Uniform Fee $25.00 $125.00 Total Due at Registration DATES: Sunday October 4th – After Morning Masses (in Church Hall) Sunday October 11th – After Morning Masses (in Church Hall) Sunday October 18th – After Morning Masses (in Church Hall) Sunday October 25th – After Morning Masses (in Church Hall) Registration closes November 1st NO, EXCEPTIONS!!
PARTICIPANTS WILL NEED THE FOLLOWING FOR PROOF OF AGE AND RESIDENCE AT THE TIME OF REGISTRATION: -Birth Certificate -Utility Bill (SAWS, CPS, or AT&T) or Parishioner’s envelope Participants must either be registered parishioners at Prince of Peace or live within the Diocesan established boundaries.
REFUND POLICY Reason for Refund Refund Amount a) Player drops prior to Commitment night (Nov. 19, 2009) FULL Refund less $4.00 form fee b) Player drops between Commitment night (Nov. 19, 2009) ½ of fee collected and the league play begins. c) Player drops after league play begins No Refund d) Team does not form FULL Refund NOTE: These fees are to be refunded as soon as possible but no later than two weeks after any of the above situations occur.
